Autism Service Dog Application

National Service Dogs is currently accepting application requests for children living in Canada between the ages of two and ten that have been diagnosed with autism or other related PDD; some restrictions may apply.

If your child meets the above criteria, please complete our online application request form and a staff member will be in contact within one week.

Please note: If you reside outside of Canada, please visit Assistance Dogs International at www.assistancedogsinternational.org for a listing of reputable service providers in your area.

Apply for a Skilled Companion Dog:
National Service Dogs is currently accepting application requests from individuals within a two hour radius of our Cambridge training centre; some restrictions may apply.

Apply for a Canine Assisted Intervention Dog:
National Service Dogs is currently accepting application requests from individuals/facilities within a three hour radius of our Cambridge training centre; some restrictions may apply.Please contact Tamara at tamara@nsd.on.ca or 519-623-4188, ext. 18 to discuss receiving an application.

Apply for a Companion Dog:
